Here is the criteria to consider when choosing students to audition for a Scholarship

Give the completed for to your teacher
Type in the information. Hand writing is often too difficult to decipher.
DO NOT SEND ADDITIONAL ATTACHMENTS
Deadline for packets to arrive in Philadelphia is January 27, 2012

This form accompanies all the Student Scholarship Registration forms
Only one copy is needed per packet.
Print or type in black ink

The initial list should be available early to mid-Novemer
Additional Schools will be listed as received.

The deadline has been extended. All forms should be completed and mailed to ARRIVE (at Penn State Abington) no later than February 13, 2012.
